American Association of Diabetes Educators Outstanding Chapter of the Year Award
PURPOSE
To recognize an AADE chapter that has demonstrated leadership, growth, community service and support of the goals of the national organization.
AWARDS
The award consists of a $1000 gift to the Chapter and a commemorative plaque. The award is generously sponsored by the BD Consumer Health Products. The award will be presented at the AADE Annual Meeting & Exhibition.
ELIGIBILITY
•All chapters that have been granted AADE chapter status (as defined in the AADE Bylaws), are in good standing, and have been active for at least six months prior to the application deadline are eligible.
•Chapters that have won this award within the last two years are ineligible.
ENTRY REQUIREMENTS
•Application must be made by a chapter officer.
•If applying for the Outstanding Chapter of the Year Award, a representative of the chapter will be required to make a brief presentation at the Annual Meeting Chapter Sharing Session.
•Application should reflect activities from January 2009 through December 2009.
•Application must include the Chapter’s Mission and Vision.
•Applications must be submitted in MS Word – Times New Roman 12pt font as an email attachment by March 1, 2010. Applications received after the deadline will be disqualified.
